The Inpatient Care Coordinator is responsible for supporting the process that manages all hospital and skilled nursing admissions, transfers of patients, concurrent review, and discharge planning. The goal of the Inpatient Care Coordinator is to effectively support the in-patient management process by assuring the right information is provided timely to the clinical administrative management team, hospitalists’ and case managers. Responsibilities:Responsible for prompt and accurate daily entries of acute, skilled and out-of-network admissions for assigned group.Ensures an accurate and timely daily census is available to the clinical team by 10:30AM daily. Responsible for obtaining bed assignments when requested and connecting OON admitting physicians to hospitalist.Ensure accurate and timely entry of; admit date/discharge diagnosis codes, LOC and discharge dispositions, into the network database daily.Captures the essential transition of care elements during clinical rounds and documents in system daily.Verify eligibility with internal systems and website or with health plans if necessary. Effectively coordinate with Hospitals for timely OON patient transfers.Request medical record information for health plan chart review and fax to appropriate case manager upon all admissions.Assist claims, QM, UM, CM departments with obtaining medical records from physician’s office upon request.Provides essentials clerical and telephonic support to designated team clinical staff.Responsible for ensuring timely ambulance authorization entries for assigned entities.Foster positive interaction and relationships with all internal departments as well as cultivating positive working relationships with external contacts Conducts timely and effective transitions of care activities under the guidance of team nurse.Promotes a pleasant and team focused professional behavior.Perform other duties as assigned.
- High school degree or equivalent
- Minimum 2 years in the managed care setting preferred
- Computer skills with Microsoft and Excel database programs
- Strong interpersonal skills
- Organized and detail oriented
- Ability to independently generate correspondence
- Physical guidelines include the ability to sit for long period of time; also stooping, bending and stretching for files and supplies, occasionally lifting of up to 30 pounds
