Personal Service Coordinator II (PSC II)
The Personal Service Coordinator II provides direct client community support and case management services to assigned clients. Employee with maintain a caseload of approximately 15 people for 30-60 days as they work to transition our members from a hospital setting back into the community.
Most linkages are to connect to outpatient mental healthcare facilities. Provides education and support to clients and their families. Collaborates with other services and agencies such as the Conservator's Office, Public Defender's Office, Child Protective Services, Probation Department and County Mental Health Services.
-Must have Bachelor's degree in behavioral health care (psychology, sociology, social work, forensic psychology, etc.
-Preferred experience working with individuals that have severe mental illness (SMI) for at least one year with Case management experience preferred
-Experience in providing intensive case management and intensive community supports linkages
-Sensitivity to multi-cultural populations and issues is required
-Must have a valid California driver's license, a DMV clearance for insurability, a vehicle, and proof of insurance
-Applicant must be finger printed and receive clearance from the Department of Justice.
Location: Near Old Town right by I-8E and I-5N
$14.88 per hour; Monday Friday; 8:00am 5:00pm; Full medical, dental, vision + many more benefits!!
