Job Posting: The Integrative Medicine Clinical Pharmacist is responsible for providing ambulatory pharmaceutical care services using approved protocols in collaboration with physicians. Includes medication therapy management and providing patient education. Assesses appropriateness of herbal or dietary supplements in conjunction with other medications. Optimizes therapeutic outcomes and promotes safe, cost-effective chronic disease medication use. Performs these duties in the clinical areas assigned.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
Upon receiving a patient specific written order for management under a specific protocol, the pharmacist develops a treatment plan, which may include drug therapy and lifestyle modifications
Under protocol, initiates, adjusts, monitors, and recommends changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, review of drug profiles and the patient medical record.
Identifies, resolves, and prevents potential and actual medication-related problems including untreated indications, improper drug selection, sub-therapeutic dosage, overdosage, failure to receive medication, adverse drug reactions, drug interactions, and medications without indication.
Evaluates herb/supplement and drug/disease interactions.
Recommends evidence-based integrative medicine options as appropriate for patient-specific needs
Coordinates and interprets laboratory monitoring of drug therapy, including serum drug levels, electrolytes, and other drug-related laboratory testing.
Provides patient monitoring for the purpose of evaluating and optimizing patient outcomes, drug utilization and patient compliance, detecting and/or minimizing adverse drug reactions, eliminating unnecessary drug use and duplication.
Provides patient education related to the individual’s disease state, medication side effects, and reasons for proper compliance.
Counsels patient on proper administration and side effects of recommended dietary and herbal supplements.
Assures ongoing performance improvement of the pharmaceutical care services provided.
Provides education to providers and staff via presentations, newsletters, formal utilization reviews, and one-on-one interactions.
Developes dietary and herbal supplements patient education materials in collaboration with other providers.
Evaluates supplement manufacturers and suppliers using FDA Good Manufacturing Practices and guidelines for safety and potency.
Responsible for maintenance of on-site supplements, not limited to ordering, stocking, and inventory.
Responsible for training pharmacy interns, residents and students rotating through CSMG.
Actively participates in electronic medical record updates and new developments as pertaining to medications/herbals/supplements.
Performs other duties as required.

EDUCATIONAL REQUIREMENTS AND/OR PREFERENCES (Licensure, Registration, Certification, Experience, Other):
Current California Pharmacist license required.
Successful completion of a pharmacy practice residency or ambulatory care residency, or equivalent clinical experience in direct patient care delivery.
Advanced residency, fellowship or board certification in Integrative Medicine, preferred.
National Provider Identifier (NPI) required.
A doctor of pharmacy (Pharm. D.) degree preferred.
American Heart Association (AHA) approved Basic Life Support (BLS) certification must be obtained within the probationary period.
Comprehensive knowledge of medical terminology as related to drug usage.
Comprehensive knowledge of common disease states and pathologies and ability to apply current pharmacological and biopharmaceutical principles.
Ability to comprehend patient medical management plans.
Ability to comprehend and interpret scientific journals.
Familiarity with national and institutional medication and supplement databases.
Ability to extract pertinent information from the patient or other sources and relate it to the medical management plan.
Possess excellent verbal and written communication skills.
Ability to serve as preceptor for teaching/training program.
Ability to give advice on and monitor the use of drugs in medical emergencies.
Knowledge of laws, regulations and standards relative to Institutional Pharmacy practice and prescription dispensing.
Knowledge of FDA recommendations in regards to supplements and familiarity with GMP and DSHEA standards.
Ability to prioritize workflow.
Ability to recommend, develop, and expand programs and services to meet the therapeutic needs of the patients.
Ability to learn and adapt to the use of automated information systems in daily practice.
Demonstrated competency in pharmaceutical calculations and pharmacokinetics.
