For more information on our philosophy and services, please tour our website. www.jfsla.org As part of a multi disciplinary team, the Case Manager / Housing Specialist advocates for, initiates and implements all aspects of a client’s case plan concerning access to required resources, services and opportunities according to established agency standards. QUALIFICATIONS:
Bachelor’s Degree in Social Work or related field or equivalent experience. Minimum of 2 year experience working with victims of domestic violence in a shelter setting. Demonstrate ability to develop, coordinate, document effectively and engage clients to create and implement their case plans. Ability to communicate effectively, perform crisis intervention, mediation and motivational interviewing strategies. Familiarity with community, housing, employment and psychological resources, and the ability to effectively link clients with available opportunities. Ability to perform community presentations, perform outreach, and build partnerships within the community. Be culturally sensitive and able to work with a multi cultural client/customer population; ability work in a non-judgmental manner and foster positive outcomes; and ability to work as part of a team and maintain professional boundaries. Intermediate computer skills including familiarity with Microsoft office, including Excel 40 hour domestic violence training required. Bi-lingual (English/Spanish) preferred. Valid driver’s license, current auto insurance and registration Must pass a criminal background check (Live Scan Fingerprinting) prior to beginning employment RESPONSIBILITIES/ESSENTIAL FUNCTIONS:
Manage case plans of shelter program clients, including: keeping statistical records, assisting clients with establishing their goals and personal budget, monitoring progress toward goals and assisting with activities related to the case plan Assist clients with employment readiness and permanent housing Assess housing barriers of individuals and families to determine housing and service needs Provide mediation and advocacy with landlords on behalf of clients to assist with obtaining/maintain housing Provide follow up services and support to help maintain stability of housing and independence Responsible for provision of shelter coverage, when needed, and assists with all aspects of daily shelter service delivery Act as liaison to and attend grant related meetings or other meetings as assigned Other duties as assigned
