This position is located in the Primary Care Health Care Group at the VA Long Beach Healthcare System in Long Beach, California. The incumbent selected for this position will be under the direct supervision of the Transition and Care Management (TCM) Program Manager/Nurse Supervisor at the VA Long Beach Healthcare System’s Veterans Transition Center. The primary responsibility of the TCM case manager is to coordinate care and services for ill and injured transitioning service members and Veterans and others in need of case management services. He/she utilizes the nursing process and evidence-based practice to collaborate with multiple interdisciplinary team members to include but not limited to members of the Patient Aligned Care Team (PACT), Mental Health, Traumatic Brain Injury, and Specialty services throughout the medical center and in the community in developing a case management treatment plan that is patient-driven and holistic in assisting the service member, Veteran and their significant other/caregiver as they transition from active duty to civilian status. The TCM case manager will apply the standards of case management practice that emphasizes a collaborate process that assesses, advocates, plans, implements, coordinates, monitor, and evaluates health care options and services in order the meet the needs of an individual patient. The TCM case manager is responsible for coordinating patient care delivery to ensure quality care according to established standards of care/practice, policy and procedure. The specific patient population to whom case management services will be provided includes service members & Veterans who served in the Post 9/11 era on or after September 11, 2001; served in combat deployments in support of Operation Iraqi Freedom (OIF), Operation Enduring Freedom (OEF), Operation New Dawn (OND) or Operation Inherit Resolve (OIR); active duty service members directly referred from a Military Treatment Facility (MTF) ; and outpatient service members who present to the VA Long Beach Healthcare System seeking health care. The TCM case manager will be working directly with many transitioning service members and Veterans who may suffer from multiple complex health and mental health problems, including but not limited to traumatic brain injury (TBI), amputations, burns, loss of vision combat stress and post-traumatic stress disorder (PTSD), military sexual trauma (MST).
Work Schedule: Monday- Friday, 8:00AM to 4:30PM
