IBH is searching for a Case Manager to review the client's history, identifies needs, and gathers information from other sources. Completes a Case Management Assessment. Collaborates with the client and assigned counselor to develop an individualized Case Management Plan (CMP). Provides services in accordance with the CMP, including empowering the client with information and possible referrals to identify, understand and utilize community services or programs. Advocates on behalf of the client to assist in accessing services or programs. Reassesses the CMP with the client in coordination with the assigned counselor to ensure client needs are identified and assessed. Builds strong positive relationships with clients, IBH treatment staff, and with service providers throughout the community. Maintains complete and accurate electronic health records in accordance with Agency, Federal and State requirements.
Qualified candidates must possess a Bachelor's degree in social work or related field or have an equivalent combination of education and experience. Experience in social service, customer service, non-profit environment, and recovery-related programs utilizing the 12 Steps of Alcoholics Anonymous. Must have strong computer skills to include Microsoft Office (Word, Excel, Outlook Email) and previous electronic health record experience.
